Player Checklist

First, fill out this short online form.

Next, gather the following documents and paperclip them in the order below, to turn into your team manager by the requested date.

1) Two recent photos (no hat, no sunglasses). Write your name, team name and TIN (team identification number) on the back of the photos. Maximum size: 1" wide by 1.25" tall.

2) Check payable to SF Vikings League for $70.00 (recreational) or $90.00 for Upper House.

3) Print two copies of CYSA Membership form 1601 signed in ink.

4) Proof of birth -copy issued by the govenment (we can not accept hospital or baptismal certificates). If the player played in Vikings League (not Microsoccer) last year and you already submitted a birth document you do not need to submit one again.

5) Print and sign the Spectator code of conduct.
