Player Checklist
player instructions These instructions apply new and returning players. First, fill out this short online form. Next, gather the following documents and paperclip them in the order below, to turn into your team manager by the date that your manager sets for you. 1) Two recent photos (no hat, no sunglasses). Write your name, team name and TIN (team identification number) on the back of the photos. Maximum size: 1" wide by 1.25" tall. Really not sure about photos? Hand a few off to your team manager and we will help select what will work. 2) Check payable to SF Vikings League for $70.00 (recreational) or $90.00 for Upper House. 3) Print two copies of CYSA Membership form 1601 and sign both in ink. 4) Proof of birth -copy issued by the government (we can not accept hospital or baptismal certificates). If the player played in Vikings League (not Microsoccer) last year and you already submitted a birth document you do not need to submit one again. 5) Print and sign the Spectator code of conduct. 6) Turn in this paperwork to your TEAM MANAGER who will review it before submission to Vikings League office. This packet may be mailed or delivered to Vikings League located at 2521 Judah Street, San Francisco, CA 94122. The team manager will be contacted when the player pass is ready for pickup. S/he may in turn, call the parent to arrange pickup from Vikings. Please do not call or email asking if the pass is ready, we have a record number of teams and are working hard to complete Fall season prep.
