Team managers must complete the Team official risk management disclosure form and sign. Please note, this link takes you off site. After you click the submit button, the 1628 form will autopopulate. Please print two, sign both.

Coach registration:

First, fill out this short online form.

Next, gather the following documents and paperclip them in the order below, to turn into your team manager by the requested date.

1) Two recent photos (no hat, no sunglasses). Write your name, team name and TIN (team identification number) on the back of the photos. Maximum size: 1" wide by 1.25" tall.

2) Coaches must complete the Team official registration and risk management disclosure form and sign. Please note, this link takes you off site.Under organization, select from the drop down menu: SF Vikings YSL (010201).. After you click the submit button, the 1628 form will autopopulate. Please print two, sign both and give to the team manager along with any applicable articles listed. If you coach multiple teams, print an extra(s) for those teams.

3) Copy of your F license or above. Or, if you need to take a clinic, proof your clinic spot is secured. Lost your license copy? Apply for duplicate through CYSA.

Volunteer Assistant Referees (VAR):
U9 - U14 rec teams need to register a Volunteer Assistant Referee. (no Forms or photos are required).
