FALL 2012 ALL TEAMS FALL REGISTRATION DETAILED CHECKLIST
- STEP 1. Online Intent to Register
- Complete Online Intent to Register
Important! Your Team Name must be unique for Fall. Consider in advance a way to distinguish your team and the teams within your school or club. Many mascot names are common, so don’t use just “Dragons",” “Bears,” “Hawks,” "Lightning" etcetera. Also don’t just use the name of your school or soccer club. Add a prefix or a suffix to your main name to make it unique. To comply with CYSA rules, at Registration the League may adjust your team name to make it distinct.
- Examples:
“Dragons” turn into “FC Dragonslayers”
“Rooftop” turn into “Rooftop Bees” and “Rooftop Stingers” etc.
“SF Glens” turn into “SF Glens Violet,” “SF Glens Black” etc.
- Fill out CYSA Team Risk Management Disclosure Form.
Team managers handle sensitive documents. In order to protect the children, all team managers must complete the online every year. Please note, this link opens a new window. Remember to use your legal name. Under organization, select from the drop down menu: SF Vikings YSL (010201).
Important! Please print or write down the 6-digit disclosure number at the end of the form and bring it to Vikings when you come to turn in your paperwork. We will need this confirmation number to complete your manager registration. If you're sending someone else to help with the team registration appointment, please provide your legal first and last name to us as well as your MOBILE PHONE.
- Note that this information (6-digit disclosure confirmation number, legal first and last name, mobile phone) is required for ALL STAFF on your team. New Coaches will also need to submit a photo and a copy of their license or their confirmation of coaching clinic registration.

- STEP 2. Online Pre-Registration
- Register with Vikings as a Manager in Reg 2.0.
All team managers need a registration account before they can use our online system. Returning managers may simply login. New managers, please apply for a manager account. Use your email address and password (chosen when you apply) to navigate the team manager section of this module.
- STEP 2A. Sign up Your Team(s) to get Team Identification Number(s) (TINs)
The first step in the signup is to provide some team information and get a TIN.
- >> ALL U8 & U9 TEAMS should choose Divison = Rec (even if you're planning to play in the competitive bracket of those age groups).
- >> ALL U10+ COMPETITIVE / CCSL PREP / TRAVEL TEAMS should choose Division = Upper House (UH). Even though travel teams are not Upper House, we are working within the technology here and will sort out who our travel teams are and what additional district fees, etc. need to occur after this initial registration period.
- STEP 2B. Player Roster assignment: Populate your team screen by searching for and adding returning players to your roster. At this point, you will only add players at this time to create a roster to print as a cover sheet for your player paperwork required for your in-person visit. Be sure to doublecheck your Roster Minimums & Maximums

Completely New Teams will use Manual Roster Form
Do you have a mix of players? (ie, some returning, plus a couple of brand new players?)
Create your roster in Reg 2.0. It's important to secure your existing players since players may not play on more than one team. Print and manually handwrite at the bottom of your roster the player names you couldn't find in the system.

- STEP 2C. Staff Registration
Important! ALL STAFF : Legal First & Last Name, Mobile Number, Email, Completed Online 1628 (Risk Mgmt Disclosure)
- Coaches: Make sure your coaches complete the required Online Coach Registration before you come in to to Vikings to process your paperwork. Note that this information (6-digit disclosure confirmation number, legal first and last name, mobile phone) is required for ALL STAFF on your team. New Coaches will also need to submit a photo and a copy of their license or their confirmation of coaching clinic registration.
Note about CYSA Team Risk Management Disclosure Form: Each staff member need only complete this form one time, once per year. Under organization, select from the drop down menu: SF Vikings YSL (010201). The confirmation number received upon completion is good for all teams, in all CYSA Leagues. If you are a Manager and Coach or a manager or coach of multiple teams, your confirmation number is good for all of your teams and each of your roles (head or assistant coach, manager, trainer) on each team.
- (For Rec) Volunteer Assistant Referees (VARs): All U9 - 14 recreational teams must have a Volunteer Assistant Referee (signed up before your office visit) No team risk management disclosure required.
- STEP 2D. Player Registration
- To begin registration for each of your players, please send the player checklist to your player's parents. In order to register, each player will need the Team Name and TIN. All players will complete Online Player Registration and pay with Visa or MasterCard
.
- Since Team Pre-Reg and Player Reg are on two different sites, team managers will register for both sites, but can choose the same username and password. As well, your login will work for multiple teams. If you already have a Fall 2012 account (and you remember it), login to begin. To create a new Team Manager account click here.

- The module
will allow parents to:
- Register and pay for new and/or returning player(s)
- Create required forms for printing and wet signature
- Upload player photo

- For Team Managers who would like to take care of the pictures for all their players, you have two options:
- Bring in a USB drive with all player pictures labeled firstname.lastname.jpg
- Upload the pictures directly to the player registration site [ link coming soon ]
- Collect all required items for each player to bring to your appointment at Vikings.

What to bring to your office visit

PAPERCLIP EACH PLAYER'S PAPERWORK, PLEASE. NO STAPLES!!

1. Photo (no hat or sunglasses) - Players can upload digitally, team manager can bring in USB drive with all player photos, or if print, make sure there are 2 copies (labeled with player name, team name and TIN on the reverse)

2. RECEIPT. This is the final page of the player registration process and clearly indicates that your player's online registration is complete and whether there is credit card payment confirmation, financial aid requested or payment made by coach/club. |
|
3. Two Signed CYSA Insurance Form 1601 Forms with original signature (sign them using blue ink pen). No scans or faxes - these need to be ORIGINAL and can be printed from the Player Registration module or completed by hand if team coach or manager supplied the forms to players separately. |

4. ALL U8 PLAYERS & ALL NEW VIKINGS PLAYERS & ALL NEW COMPETITIVE PLAYERS : Proof of Birth : copy issued by the government (we can not accept hospital or baptismal certificates). If your U9+ player registered with the Vikings League (NOT Microsoccer) last year and already submitted a birth document you do not need to submit one again.
Is this your first year in Upper House/CCSL Prep? If this is your team's first year in Upper House / Select / Travel, collect a birth doc for all your players. Just one or two newbies to your returning Upper House team? Just need the birth docs for those new players.
Does your returning team have a mix of players? (ie, some returning, plus some brand new players?) Just need the birth docs for those new players.

5. Spectator Code of Conduct - signed by parent, TIN filled in. (This document can be printed from the Player Registration module or completed by hand if team coach or manager supplied the form separately.) |

Appointment Expectations
At your office visit, we will time & date stamp your paperwork if it is complete. That time date stamp will be worked into part of your team priorty for field practice requests. (We also take into consideration sportsmanship.)
REMEMBER : TOP TEN reasons that team registration gets delayed:
- Forgetting to print & sign TWO copies of each player’s 1601 form.
- Parents scanning and emailing 1601 forms rather than printing them out and signing them in INK.
- Accidentally putting a mother’s birth YEAR in the boxes which call for mother's birth MONTH and DAY.
- Blank “Team/Coach Request” on the 1601.
- Submitting hospital certificates or baptismal certificates rather a government-issued PASSPORT or BIRTH CERTIFICATE. (A photocopy will suffice, but it has to be a government document.)
- Player photos that crop hair or are too dark. (Ideal photo styles are or are akin to passport or school portraits.)
- Showing up for your In-Person visit without complete paperwork for enough players to make the roster minimum.

- A player has tried to sign up for TWO teams (such as their school team and a Select team.) A player can only be on one team.
- Showing up for your In-Person visit without complete paperwork for team staff.

- STEP 4. Finalizing Team Logistics
- Complete Team Census (not live yet)
- Complete Schedule Request (not live yet)
- Complete Park & Rec Online Field Request Form.
(This step must wait until after you receive your team's Practice Field Priority Code from the League and Park & Rec readies their request system.) Deadline: June 15. (not live yet)

Anatomy of a Team Identification Number (TIN) This 8-digit alphanumeric ID is critical for your communication with the League, and your unique code is used in everything from registration, roster management and scheduling. You must reference your TIN in every email and phone call.


Fall 2012-13 Team Min/Max Chart
At no time may the team fall below the minimum number of players or exceed the team maximum number of players. Coaches and team managers, please use the chart below to determine what the team minimum and maximums are for your playing age group. Please note that these numbers are subject to change each year without prior notification.
