Have the coach or former manager email us new manager's legal name, email and phone number (be sure to include Team Name and TIN number)
Email us your new spring manager's legal name and electronic CYSA Team Risk Management Disclosure (Form 1628) 6-digit confirmation number. Remember to include your TIN and team name so we know how to match up the information.

For Spring: List of important Spring dates
Remind your coaches to attend the mandatory coaches meeting to pick up player and coach passes. You'll need them for game day.
RECREATIONAL TEAMS

In house registration dates and times with SFPAL will be January 30th-February 10th, Monday-Friday, 9:30am-4:30pm.
Tuesdays, January 31st and February 7th, PAL will extend their hours till 7pm.
Feb 6th: Annual General Meeting for all Vikings-registered teams.
March 17th: Season begins
May 20th: Last day of season

UPPER HOUSE TEAMS
December 9th: All teams have already stated their “intent to play” or not.
February 1: $200 due to “San Francisco Vikings League” to hold your spot.
Feb 6th: Annual General Meeting for all Vikings-registered teams.
February 24: roster change/paperwork deadline. If no changes, do nothing. If you have roster changes, email the office. To drop a player, you only need to forward an email from the parent – you don’t have to fill out the drop form.
March 7th: Mandatory Coach meeting and additional fees due to cover the cost of referees – $300 for U10/U11, $360 for U12 and above.
March 17th: Season begins
May 20th: Last day of season

Team Registration Requirements
All paperwork should be reviewed by the manager first for completeness. Kindly make certain all team identifying numbers/names are on all documents. Check if player is signed up online (this does not mean paperwork has been submitted)
Complete paperwork includes the following for each player:
- Two recent photos (Make sure we can see the above the hair and below the chin, and that the player is facing forward.). You can bring them electronically or have physical pictures. If you've only printed copies, write player's name, team name and TIN (team identification number) on the back of the photos. Maximum printed size: 1" wide by 1.25" tall (like a passport picture).
- Check payable to SF Vikings League for $70.00 (recreational) or $90.00 for Upper House.
- Two completely filled CYSA Membership form 1601 both signed in ink. No scans or PDF's, we need original signatures.
- Proof of birth -copy issued by the government (we can not accept hospital or baptismal certificates). If the player played in Vikings League (not Microsoccer) last year already submitted a birth document you do not need to submit one again.
- Signed Spectator code of conduct.
When you are satisfied the paperwork is perfect, you may mail them to us. Address the envelope to:

Fall 2011 Team Min/Max Chart
At no time may the team fall below the minimum number of players or exceed the team maximum number of players. Coaches and team managers, please use the chart below to determine what the team minimum and maximums are for your playing age group. Please note that these numbers are subject to change each year without prior notification.
