Players With Team - How To Register

These instruction are for players who have a team to play on. If you are a player (or parent of a player) and are looking for a team, go to How To Find A Team. If you are on a Jewish Community Center (JCC) Team, do not follow these instructions, contact the JCC.

Step 1 - Get the TIN and Team Name From Your Coach
After you find a team for your child, you will need to get the Fall 2008 Team Identification Number (TIN) and team name from their coach. The correct number is essential to making sure that your child ends up on the right team.

Step 2 - Register The Player On-Line
Ask your coach if they are planning to register your child on-line or if you need to. If they request that you do, click on the Players With Team link and register your player on-line. You will need the child’s name, birthday, mothers birthdate (month/year only), and an e-mail address.

Before you submit it we recommend that you print a copy of what you send for your records. If a player does not have access to the Internet, anyone associated with the team can complete this form for the player or you can visit the public library or local college library.

Step 3 - Complete the CYSA Form
The primary contact or coach of your team should provide you with an original CYSA registration form #1601. (No photocopied or faxed versions will be accepted). Please make sure that the child's name matches their birth certificate/passport exactly (no nicknames). Please be sure the parent/guardian signs in all places indicated.

Step 4 - Sign the Spectator Code of Conduct form
The primary contact or coach should provide you with this form. It is also available to download and print by clicking here for the English version or aqui for the Spanish version.

Step 5 - Give CYSA Form, Birth Certificate, Fees, and Spectator Code of Conduct form To The Coach or Team Manager
Let the Coach/Manager know that you completed on-line registration and provide your coach with each of the following:

a.  PICTURES - a 1”x 1.5” current school or passport quality/size picture of the face of each player and coach/team manager on your team. Write the player name, team name, & TIN # on the back. No hat or sunglasses.
Small photos, large photos and unclear images will be rejected.
Player/adult must be looking at the camera.
Do not truncate ears/head or chin.

b. CHECK - Payable to the SF Vikings Soccer League in the amount of $70. Please write team name and TIN # and player last name in the memo field. Returned check fee of $25.00 applies.

c. CYSA FORM 1601 California Youth Membership form (Rev. 5/2005 or later) signed by a parent with INK where indicated.

d. BIRTH CERTIFICATES/CURERNT PASSPORTS - A copy of your child’s birth certificate or passport (these will not be returned). No hospital records will be accepted.

e. SPECTATOR CODE OF CONDUCT FORM ( (Spectator Code in Spanish) for each player, signed by parent/guardian with the signature facing up.

f. Return the above items (clipped together in the order above) to the team coach or manager before their registration appointment. Do NOT bring them to the Vikings office because the coach or team manager is to submit the entire team's paperwork all together at the same time. If their registration appointment has passed, please communicate with the manager on how you will get the forms to our office.

August 14, 2008 is the deadline to submit complete paperwork for players to be eligibile for the first game on September 6.
