All Vikings teams are required to have a "Primary Team Contact" or Team Manager. This person can be a parent or also be the coach of the team. If the Manager is also planning to coach, please follow the instructions for COACHES (below). In order to register only as the Manager, not coach, please do the following:

Submit Team Official Risk Management Disclosure:

 CYSA FORM #1628. (link to CYSA site for an electronic submission). Fill out the form online, hit submit button then print 2 copies of the result form. Sign both copies of the form. If you are coaching more than one team, please only submit electronically one time, but make an additional copy of signed form #1628 for each team.

Registering as a Coach

All Vikings teams are required to have at least one registered F-licensed coach on the field for every game. We highly recommend two coaches to avoid game forfeiture and fees in the event the head coach is unable to arrive to every game on time.

There can only be a maximim of two coaches on the coaching side of the field at anytime during games. Any adult planning to give instruction to the players during practices and/or games must register with the league. To register, please take the following steps:

1) Obtain at minimum of an F coach license by attending an F clinic. You can take an F clinic hosted by Vikings or another league. Clinics fill up fast so register early. If you are unable to attend one of the clinics hosted by Vikings, you can find a list of clinics being offered in Northern California at the CYSA website. If you already have a license but have misplaced it, you can request a copy of coach license from CYSA.

2) Signup online as soon as you get your new 2009 TIN (team identification number) so we can contact you via email early in the process.

3) Submit Team Official Risk Management Disclosure:

 CYSA FORM #1628. (link to CYSA site for an electronic submission). Fill out the form online, hit submit button then print 2 copies of the result form. Sign both copies of the form. If you are coaching more than one team, please only submit electronically one time, but make an additional copy of signed form #1628 for each team.

4) Submit the following to your team's manager before their registration appointment in May. (If appointment is for spring rec play, bring the paperwork directly to Vikings at least one week prior to sign-up at PAL):

a. Two signed CYSA #1628 Forms as described in 3) above.
b. A copy of your F license (the business size card you recieved when you completed the F Clinic).
c. One photo (TWO if Upper House or Club Team) with team name and TIN written on back. If you are planning on being on the sideline during games, submit a passport size and quality photo so a game pass can be made for you.
No sunglasses, hats, costumes or funny faces. No plastic coating. If you are coaching more than one team, we need one photo per team. Coach passes are only valid for the team stated on the coaching pass. We can not accept copies from passports or drivers licenses because of the wavy lines.

Additional Requirements:

1) Each team is required to submit a Team Census by the day before your team's registration appointment. Only one is required per team. Please do not submit multiple forms, although an amended census is acceptable.

2) Attend the Mandatory Coaches Meeting (September) to receive important items and information for games. Dates/times can be found at the registration link.

3) Make sure at least one coach on your team is wearing a red hat (received at the coaches meeting) to every game.

4) Make sure at least one coach from your team submits a Coach Report by Sunday night after every game.
