Player Checklist

Returning players, sign up through the online link provided by your Team Manager.

New players to Vikings League (this would include kids moving up from Micro ) Fill out this short online form.

Next, gather the following documents and paperclip them in the order below, to turn into your team manager by the requested date.

1) Two recent photos (no hat, no sunglasses). Write your name, team name and TIN (team identification number) on the back of the photos. Maximum size: 1" wide by 1.25" tall, if no Jpeg is provided. Remember to rename the file to include your child's name.

2) Online payment for $70.00 (recreational) or $90.00 for Upper House plus any donation.

3) Print two copies of CYSA Membership form 1601 signed in ink. Tip: Fill out the online form, print two and sign BOTH in blue ink. No scans, faxes or pdf's.

4) Proof of birth -copy issued by the government (we can not accept hospital or baptismal certificates). If the player played in Vikings League (not Microsoccer) last year and you already submitted a birth document you do not need to submit one again unless your child is moving up to competitive soccer (Upper House/Travel team)

5) Print and sign the Spectator code of conduct if not provided online.
